About the role

The Wellness Nurse-Registered Nurse (RN) reports directly to the Director of Nursing and is responsible for monitoring the health and wellbeing of residents, providing leadership support, and assisting with resident care and services. This role includes medication care administration, communication with resident support services and families, and maintaining clinical quality assurance while supporting all elements of wellness.

Schedule

Monday-Friday, with on-call responsibilities. Ability to work weekends, evenings, and flexible hours as needed.

Responsibilities

  • Identifies ongoing needs and services of residents through the assessment and Personalized Service Plan (PSP) process.
  • Ensures proper follow-through and documentation for residents with changes in clinical care needs.
  • Assists the Wellness Director with completing the clinical sections of monthly wellness visits and PSPs to accurately reflect resident status.
  • Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out.
  • Assists with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
  • Assists with direct resident care as needed.

Requirements

  • Must be a Registered Nurse (RN).
  • A minimum of one year of experience working with elders or disabled individuals, preferred.
  • Demonstrated leadership capabilities.
  • Knowledge and experience in the Assisted Living industry and Dementia care or Skilled Nursing, preferred.

Skills

  • Understanding of infection control procedures.
  • Proficient in using MS Office.
  • Able to implement Personalized Service Plans.
  • Understands advanced directives and end-of-life care.
  • Embraces the assisted living philosophy and understands nursing function in assisted living, quality of care, and quality of life objectives.
  • Demonstrated knowledge of state regulations and proficiency in using state audit compliance tools.
  • Proficient in time management skills and adherence to deadlines.
  • Exceptional grammar and documentation skills.
  • Understands basic concepts of risk management.
