Unit Care Coordinator (Registered Nurse/RN) → Care Coordinator (Registered Nurse/RN)

Qualifications

• Nursing diploma (associate’s or bachelor’s degree)
• Active RN license in applicable state
• Preferably one year of geriatric nursing experience
• CPR certification (obtained upon hire or during orientation)

Responsibilities

• Supervise and coordinate nursing staff and care activities
• Maintain accurate and timely documentation
• Manage patient care plans to promote physical, mental, and psychosocial well-being
• Administer medications as ordered and respond to emergencies
• Foster positive relationships with patients, families, and team members
• Ensure safety, confidentiality, and compliance with policies
• Assist in patient evacuation when needed
• Exhibit professionalism, integrity, and excellent customer service

Skills

• Strong leadership and decision-making skills
• Knowledge of post-acute care practices, laws, and regulations
• Ability to communicate effectively and work collaboratively
• Physical ability to perform job functions including mobility and manual tasks
• Proficient in English communication skills
