About the role

The RN Unit Care Coordinator oversees and manages patient care on their unit, ensuring compliance with laws, regulations, and standards.

Qualifications

  • Nursing diploma (associate’s or bachelor’s degree)
  • Active RN license in good standing
  • Preferred geriatric nursing experience
  • CPR certification (obtained during orientation if not already certified)

Responsibilities

  • Lead and supervise nursing staff and CNAs
  • Coordinate and manage patient care plans and services
  • Ensure accurate and timely documentation
  • Administer medications as ordered
  • Respond to emergencies and ensure safety
  • Foster positive relationships with staff, patients, and vendors
  • Maintain confidentiality and adhere to policies and codes of conduct
  • Exhibit professionalism, integrity, and teamwork

Skills

  • Advanced knowledge of practice and laws governing post-acute care
  • Ability to make independent decisions
  • Strong communication, reasoning, and judgment skills
  • Ability to handle physical tasks and work productively within a team
