UM Case Manager

About the role

As an UM Case Manager at Havenwyck Hospital, you will be responsible for managing patient care plans, coordinating with healthcare providers, and ensuring patients receive comprehensive, high-quality care.

Responsibilities

• Develop and implement care plans for assigned patients
• Communicate with healthcare providers to coordinate patient care
• Evaluate patient progress and make adjustments to care plans as necessary
• Ensure compliance with regulatory standards and policies
• Provide emotional support and guidance to patients and their families

Requirements

• Bachelor's degree in a healthcare-related field
• Minimum of 2 years of relevant case management experience
• Knowledge of healthcare regulations and policies
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Qualifications

• Current certification as a Case Manager (CM)
• Experience working with diverse patient populations
• Strong organizational and problem-solving skills

Pay
