About the Role

WCS Healthcare is seeking an experienced Registered Nurse (RN) with UAS Assessment experience to join a growing Home Health team serving Suffolk County, Long Island. Responsibilities

  • Conduct UAS-NY assessments in patients' homes throughout Suffolk County
  • Evaluate physical, cognitive, psychosocial, and functional needs
  • Complete accurate and timely assessment documentation
  • Determine eligibility for home care and long-term care services
  • Develop recommendations based on assessment findings
  • Collaborate with physicians, care managers, caregivers, and interdisciplinary teams
  • Educate patients and families on available services and community resources
  • Maintain compliance with agency policies and New York State regulations

Requirements

  • Active New York State Registered Nurse (RN) License
  • Current UAS-NY Certification
  • Previous UAS Assessment experience
  • Strong clinical assessment and documentation skills
  • Excellent communication and time management abilities

Preferred Qualifications

  • Home Care experience
  • MLTC experience
  • Case Management experience
  • Community Health experience

Schedule

  • Full-Time Monday-Friday 9:00 AM-5:00 PM
  • 3 days conducting in-home assessments throughout Suffolk County
  • 2 days working remotely from home
