UAS Nurse Assessor (RN)- Queens

About the Role

WCS Healthcare is partnering with a respected Home Health Agency serving Queens, NY to hire an experienced Registered Nurse (RN) UAS Assessment Nurse. This is an excellent opportunity for a compassionate and detail-oriented RN seeking a flexible hybrid schedule while making a meaningful impact in the community.

Responsibilities

• Conduct UAS-NY assessments for members throughout Queens.
• Evaluate patients' physical, functional, cognitive, and psychosocial needs.
• Complete accurate and timely assessment documentation.
• Determine eligibility and appropriate levels of care for home-based services.
• Develop care recommendations based on assessment findings.
• Collaborate with physicians, caregivers, case managers, and interdisciplinary healthcare teams.
• Educate patients and families regarding available healthcare resources and services.
• Maintain compliance with agency policies, regulatory requirements, and New York State guidelines.

Requirements

• Active New York State Registered Nurse (RN) License
• Current UAS-NY Certification
• Previous UAS assessment experience required
• Home Care, MLTC, Case Management, Community Health, or Care Management experience preferred
• Strong clinical assessment and documentation skills
• Excellent communication and organizational abilities
• Ability to work independently both in the field and remotely

Schedule

Full-Time Monday-Friday 9:00 AM-5:00 PM

• Hybrid Schedule: 3 days conducting in-home assessments throughout Queens
• 2 days working remotely from home
