About the role

This part-time, remote position provides telephone triage nursing for Assisted Living and Hospice support.

Schedule

• Every 3rd weekend (Saturday & Sunday)
• 7:00 AM – 3:00 PM

Pay

The targeted pay range is $36.00 – $41.00 per hour. Ecumen considers experience, qualifications, certifications, licenses, and education when determining new-hire pay.

Responsibilities

• Independently provide professional nursing services over the telephone for Ecumen patient and resident care.
• Accurately document symptoms/condition, assessment, advice provided, and patient/caller response.
• Follow policies, procedures, and protocols to ensure consistency, improve outcomes, and ensure satisfaction of clients and staff.
• Follow up on situations representing higher risk or need from family or staff.
• Analyze data and trends; develop solutions to improve services and outcomes.
• Design workflows, develop plans, and implement actions.
• Participate in quality improvement activities as assigned.
• Provide high-level nursing support for patient care questions at assigned site, including incident follow-up.
• Efficiently and accurately document in multiple electronic health records.
• Maintain high standards in clinical skills and professional ethics as established by the Nurse Practice Act and Ecumen.
• Perform other duties as assigned.

Requirements

• Must be licensed as a Registered Nurse in Minnesota and must maintain licensure or obtain within 60 days of hire.
• One year of care management experience in home care or hospice.
• Strong clinical and relational skills.
• Ability to analyze situations and implement solutions.
• Ability to handle multiple priorities, ambiguity, frequent change, and high-stress situations.
• Ability to communicate effectively in both verbal and written formats.
• Proficient with Microsoft Office Suite.
• Ability to follow all safety rules, regulations, policies, and procedures of the facility, including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to Know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations.

Preferred Qualifications

• Prior RN triage experience.
