Triage Nurse (RN)

About the role

The Triage Nurse (RN) assesses patients' health problems and needs, develops and executes nursing care plans, and assists in maintaining patient medical records. The Triage Nurse works in a clinic-based or telephonic environment under the supervision of clinic providers.

Responsibilities

• Evaluate patients with ambulatory diagnoses and possible acute conditions.
• Initiate treatment for common ambulatory diagnoses in consultation with mid-levels and PCPs.
• Evaluate and provide emotional, psychological, and spiritual support to patients, friends, and families.
• Educate patients, friends, and family on understanding their condition, medications, and self-care skills.
• Adhere to therapeutic standards and measure health outcomes against care goals and standards.
• Resolve patient problems and needs through multidisciplinary team strategies.
• Perform additional duties as assigned.

Requirements

• 2 to 5 years of Urgent Care / Emergency Room experience.
• Bilingual in English and Spanish.
