Responsibilities

  • Complete and maintain thorough documentation including but not limited to: Monthly Notes and Reports, 14 Day Plans, 30 Day Plans, 45 Day Plans, Social Histories, Quarterly Reports, and Annual Reports.
  • Assess the needs of foster children, and implement tangible objectives, strategies and services to address all identified needs.
  • Complete home and field visits multiple times a month for each child.
  • Attend or coordinate meetings on behalf of foster children and foster families including; FAPT, Court, FPM meetings, Treatment Teams, and VEMAT meetings.
  • Supervise visitations between foster children and their birth families, provide transportation for identified services, and make service referrals to various agencies.
  • Participate in ongoing trainings required by LCPA standards, HumanKind, and supervisor.
  • Participate in all audits, to include internal and external conduct by LCPA Licensing Inspector.
  • Use Placement/Matching protocol in the event assistant is needed for placement by the Program Manager.
  • Provide ongoing support and training to foster families.

Requirements

  • A baccalaureate degree in any field plus two years of experience in providing casework services to children and families; OR a baccalaureate degree in social work or a field related to social work including, but not limited to sociology, psychology, special education, or counseling plus one year of experience in providing casework services to children and families; OR a doctorate or master’s degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling with a student placement in casework services to children and families or one year of experience in providing casework.
  • Valid driver’s license and acceptable driving record.
  • Complete, pass and maintain all required Background checks and drug test.
  • Must be able to respond to on-call emergencies within an approved amount of time as determined by Program Manager.

  • Ability to handle and manage crisis situations appropriately.
  • Be able to maintain confidentiality.
  • Comply with LCPA standards, HumanKind and HK TFC policies and procedures, and remain up to date on any changes.

Pay

$23.25 - $26.50 per hour + Stipend for after hour emergency rotation.
