Treatment Coordinator

About the role

The Treatment Coordinator plays a critical role in our acute medical detoxification program by ensuring safe, seamless, and supportive transitions for clients completing their initial withdrawal management. Recognizing that detox is only the first step in sustainable recovery, you will work closely with clients, their families, and the multidisciplinary clinical team to build robust, individualized continuing care plans. Your primary objective is to bridge the gap between acute stabilization and long-term treatment, minimizing the risk of relapse and readmission.

Responsibilities

• Create a supportive, non-judgmental environment to build rapport, and utilize motivational interviewing to assist and motivate individuals experiencing ambivalence as they navigate resources and explore treatment options.
• Proactively address treatment barriers, foster interdisciplinary collaboration, and coordinate local and statewide resources to ensure intentional, seamless transitions from withdrawal management services to the next identified steps of care.
• Provide direct case management to clients enrolled in the withdrawal management program, including linkages to assessments, mental health evaluations, housing, community resources, recovery-oriented supports, identified treatment programs, and discharge planning.
• Assist clients in identifying CMNA goals, developing relapse prevention plans, and navigating Behavioral Health related services.
• Function as a role model to clients and serve as a liaison to providers and treatment teams.
• Provide motivational interviewing, social, and emotional support, and help clients understand and employ recovery strategies.
• Maintain an active awareness of crisis support services available in the State of Alaska.
• Motivate clients through positive means, highlighting strengths and resources.
• Participate in training to improve service delivery and model/teach clients vital life skills that aid in their success.
• Advocate for clients in treatment settings and within the community, sharing lived experiences to assist in community, relationship, and skill building.
• Interact with clients in a manner conducive to their safety, dignity, privacy, and security.
• Attend staff meetings and collaborate with other staff in case staffing/review.
• Complete timely and accurate documentation in clients’ Electronic Health Records (EHR) using DAP or DJF note format, complying with state or federal guidelines.
• Demonstrate competency in documentation of Crisis Stabilization Plans, Behavioral Health and Substance Use screenings, Crisis Case Management Assessments, and Withdrawal Management Treatment Planning.
• Administer UAs and breathalyzers as needed, complete follow-up documentation, and order testing.
• Provide assistance with community outreach needs and services when appropriate and available.

Requirements

• Associate's degree in human services or related field. An equivalent combination of education and experience may be accepted.
• If in recovery, one year of continuous sobriety prior to hire and active participation in a recovery support program, such as a 12-step program.
• Must obtain CDC I and PSP III within 4 years of hire.
• Valid Alaska driver’s license and insurable under TRUE NORTH RECOVERY INC’s automotive insurance (requires driver to be at least 21 years of age).
• Continued employment is contingent upon receipt of a satisfactory state and federal background check.

Physical Requirements

This role is primarily performed in an office building with a standard office environment and controlled temperatures. When working outdoors or at a job site, the employee may be exposed to construction equipment or other moving objects/equipment. The employee is occasionally required to lift and/or move up to 50 lbs. Employee is frequently required to sit, stand, walk, talk/hear, see, read, and personally interact with others.
