About the role

La Clinica de Familia (LCDF) is a Federally Qualified Health Center with several locations in Southern New Mexico. For over 40 years we have provided quality medical, dental, behavioral health, and educational services to residents of the region. The Treatment Coordinator provides case management services to a maximum of eight (8) TFC clients and offers regular support and technical assistance to TFC parents in implementing each client’s treatment plan.

Responsibilities

• Provide case management services to up to eight (8) TFC clients.
• Support and give technical assistance to TFC parents in implementing each client’s treatment plan.
• Assist with training new TFC parents.
• Conduct home visits.
• Write home study reports.
• Demonstrate excellent written and verbal communication skills.
• Meet specified deadlines and manage time effectively.
• Interpret, comprehend, and complete delegated tasks efficiently.
• Work independently as a self‑starter and follow up on all assignments.
• Multi‑task, prioritize, and work under pressure while maintaining objectives.
• Maintain exemplary organizational skills and professional documentation.
• Maintain reliable attendance and a professional appearance.
• Contribute positively to a productive team environment.
• Maintain confidentiality and discretion at all times.
• Familiarize with all company and program policies and procedures.

Requirements

• Master’s degree from an accredited program in social work or another human‑services field; or a bachelor’s degree in social work or a related human‑service field plus two years’ experience with this population.

Pay

Up to $26.22 per hour (Non‑Exempt).
