Requirements

• Required 2+ years of experience.

Responsibilities

• Implements and monitors patient care plans.
• Maintains accurate records of patient conditions and communicates as necessary.
• Serves as a primary coordinator for interdisciplinary patient care teams.
• Carries out physician and nursing orders as directed.
• Affirms and coordinates discharge planning needs with healthcare team members.
• Works effectively in a high-stress environment.
