Job Details

Minimum Hours Per Week: 32 hours
Required Qualifications
Valid Oregon State license
BLS certification
Minimum 1 year of skilled home health experience
Reliable transportation
Covid-19 vaccination (primary series and booster)
Oasis experience
Preferred Qualifications
Experience working with adult and geriatric patient populations
Familiarity with electronic charting systems such as EPIC and Rover
Additional clinical certifications as applicable
Patient Ratio
Experience: 1:1 therapy per patient; visits up to 6 patients per day with productivity standards met.
