Travel Social Worker - $2,061 per week

Key Responsibilities

• Coordinate care and services for patients who meet Care Management criteria or require additional support.
• Conduct comprehensive assessments of patients' medical, functional, and social needs.
• Work directly with patients and families/caregivers to establish goals and appropriate interventions.
• Cookordination and support discharge planning.
• Assess patients' prior level of functioning, available support systems, and access to community resources.
• Help develop individualized plans of care with the multidisciplinary healthcare team.
• Aid in identifying appropriate care settings and continued service needs.
• Connect patients and families with community resources and support services.
• Collaborate with physicians and other members of the healthcare team on behalf of patients.
• Participate in rounds, case conferences, and multidisciplinary meetings.
• Advocate for patient needs and rights.
• Support appropriate utilization of healthcare resources and help identify strategies to reduce unnecessary length of stay.
• Maintain monitoring outcomes through data collection and variance tracking.
• Provide education regarding case management and discharge planning processes.
• Participate in multidisciplinary groups and development of guidelines.

Required Qualifications

• Active LCSW license in Montana.
• Minimum 1 year of clinical experience.
• Graduate of an accredited school of Social Work, Sociology, or related field.
• Hospital Care Management experience required.
• Strong assessment, communication, care coordination, and discharge planning skills.
• Ability to collaborate effectively with physicians and multidisciplinary healthcare teams.
• BLS/CPR required.

Preferred Qualifications

• 2 years of hospital-based Case Management experience.
• ACM or CCM certification is preferred but not required.
• Previous experience with hospital discharge planning and care coordination.
