Responsibilities
- Discharge planning and care coordination in a hospital setting
Requirements
- Master of Social Work (MSW) degree
- Basic Life Support (BLS) certification
- 2+ years of recent hospital MSW experience (within the last 3 months)
- Epic electronic health record experience
- Prior travel assignment experience (no first-time travelers)
- Licensed Clinical Social Worker (LCSW) preferred
