The position is for a Licensed Social Worker – Acute Hospital, supporting patients and families with discharge planning and care coordination in Med‑Surg and Emergency Department units. Responsibilities

• Provide comprehensive discharge planning and transition‑of‑care support.
• Assess patients' psychosocial needs and identify barriers to safe discharge.
• Coordinate post‑acute services, including appropriate community resources and follow‑up care.
• Collaborate with physicians, nurses, case managers, and other members of the interdisciplinary care team.
• Communicate effectively with patients and families regarding discharge plans and available resources.
• Maintain accurate and timely documentation in the medical record.
• Assist with care coordination and facilitate appropriate transitions from the acute‑care setting.

Requirements

• Active Licensed Social Worker (LSW) license in New Jersey.
• Previous acute hospital experience is required.
• Experience working with Med‑Surg and Emergency Department (ED) units.

Schedule

Monday–Friday, 8:00 AM–4:30 PM. Weekend rotation: rotating weekends; when working a weekend day, the Social Worker receives a weekday off.

Pay

Pay package is based on 8‑hour shifts and 40.0 hours per week (subject to confirmation) with a tax‑free stipend amount to be determined.
