About the role
Medlivo Therapy is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week contract starting September 5, 2026.
Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility setting.
- Document patient progress using EPIC EMR.
- Adhere to all safety protocols and regulations.
Requirements
- Active Physical Therapist License.
- Current BLS/BCLS Certification.
- Experience with EPIC EMR preferred.
- Bachelor's or Master's Degree in Physical Therapy from an accredited PT program.
Qualifications

Pay
Weekly Gross: $2,800.00
