Pioneer Healthcare Services is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment in San Rafael, California. This position offers an opportunity to make a significant impact in a SNF setting.
Responsibilities
- Assess patients' physical abilities and develop personalized treatment plans
- Provide therapy to improve mobility, strength, and functional independence
- Collaborate with healthcare teams to ensure comprehensive patient care
- Educate patients and families on exercise programs and injury prevention
- Monitor and document patient progress and adjust treatment plans as needed
- Implement interventions in various settings
- Participate in discharge planning and coordinate follow-up care for continued rehabilitation
Requirements
- Doctorate in Physical Therapy
- Active California Physical Therapist (PT) license
- Current BLS/CPR Certification
Pay
$2,553.49 - $2,632.46 per week
Schedule
- 13-week assignment
- Day shift, 8 hours per day, up to 40 hours per week
- On-site at a Skilled Nursing Facility (SNF)
