Travel Skilled Nursing Facility (SNF) Physical Therapist position in Eureka, California. This 13-week assignment offers 40 hours per week on an 8-hour day shift in an on-site setting.

Responsibilities

• Assess patients' physical abilities and develop personalized treatment plans
• Provide therapy to improve mobility, strength, and functional independence
• Collaborate with healthcare teams to ensure comprehensive patient care
• Educate patients and families on exercise programs and injury prevention
• Monitor and document patient progress and adjust treatment plans as needed
• Implement interventions in various settings
• Participate in discharge planning and coordinate follow-up care for continued rehabilitation

Requirements

• Doctorate in Physical Therapy
• California Physical Therapist (PT) license
• Current BLS/CPR Certification

Pay

$2,553.49 - $2,632.46 per week
