Prime HealthCare Staffing is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment in Granbury, Texas. This opportunity involves supporting residents in a skilled nursing facility setting, helping patients regain strength, improve mobility, and build confidence as they work toward greater independence. The facility is located in Granbury, a community known for its historic downtown square, Lake Granbury, and proximity to the Dallas–Fort Worth area.
Responsibilities
- Evaluate residents and develop individualized plans of care based on their needs and functional goals.
- Provide therapeutic interventions to improve strength, balance, endurance, and mobility.
- Assist residents with gait training, transfers, and functional movement to promote safety and independence.
- Monitor patient progress and adjust treatment approaches as appropriate.
- Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention.
- Collaborate with the interdisciplinary healthcare team to support quality care and positive patient outcomes.
- Complete documentation in accordance with facility guidelines and professional standards.
Schedule
40 hours per week, 8-hour day shifts. 13-week travel assignment.
