Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,150 per week

About the role

Ardor Health Solutions is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13‑week assignment in Palm Coast, Florida, starting October 12, 2026. The position entails 40 hours per week on 8‑hour day shifts, working in a SNF/Rehab setting with an adult and geriatric rehabilitation population. The therapist will help patients regain mobility, strength, balance, and functional independence following illness, injury, surgery, or hospitalization.

Responsibilities

• PT evaluations and reassessments
• Individualized plans of care
• Therapeutic exercise and strengthening
• Gait and balance training
• Transfer and functional mobility training

Schedule

40 hours per week; 8‑hour day shifts.
