Pioneer Healthcare Services is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Tinton Falls, New Jersey. The position involves assessing patients' physical abilities, developing personalized treatment plans, providing therapy to improve mobility, strength, and functional independence, collaborating with healthcare teams, educating patients and families, monitoring and documenting progress, implementing interventions, participating in discharge planning, and coordinating follow‑up care.

Responsibilities

• Assess patients' physical abilities and develop personalized treatment plans
• Provide therapy to improve mobility, strength, and functional independence
• Collaborate with healthcare teams to ensure comprehensive patient care
• Educate patients and families on exercise programs and injury prevention
• Monitor and document patient progress and adjust treatment plans as needed
• Implement interventions in various settings
• Participate in discharge planning and coordinate follow-up care for continued rehabilitation

Qualifications

• Doctorate in Physical Therapy
• NJ Physical Therapist PT license
• Current BLS/CPR Certification

Pay

$2,090.43 – $2,155.08 per week (based on 8‑hour shifts, 40 hours per week; tax‑free stipend to be determined)

Schedule

40 hours per week, day shift (8‑hour shifts, 5×8)
