Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,020 per week

About the role

CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment starting July 11, 2026, in St Charles, Minnesota.

Responsibilities

• Provide physical therapy services to patients in SNFs
• Develop and implement individualized treatment plans
• Evaluate patient progress and adjust treatment plans as necessary
• Document patient care activities and outcomes accurately
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Adhere to all regulatory and accreditation standards

Requirements

• Current licensure as a Physical Therapist in Minnesota
• At least 1 year of recent clinical experience in SNFs
• Knowledge of Medicare/Medicaid regulations and reimbursement processes
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Qualifications

• Master’s degree in Physical Therapy from an accredited program
• Passing score on the National Physical Therapy Examination (NPTE)
• Current CPR certification

Skills

• Strong clinical judgment and problem-solving skills
• Effective time management and organizational skills
• Proficiency in computer software for documentation and scheduling

Pay

The pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation), with a tax-free stipend amount to be determined. The estimated weekly payment includes hourly wages, meal and incidental expenses, and housing expenses incurred on behalf of the company.
