Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,020 per week

About the role

CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment starting July 10, 2026, in Chisholm, Minnesota.

Responsibilities

• Provide physical therapy services to patients in SNFs
• Develop and implement treatment plans based on patient needs
• Assess patient progress and adjust treatment plans accordingly
• Document patient care and treatment sessions accurately
• Collaborate with interdisciplinary teams to ensure comprehensive patient care

Requirements

• Current licensure as a Physical Therapist in the state of Minnesota
• At least 1 year of recent relevant experience in SNF settings
• Knowledge of Medicare and Medicaid regulations
• Excellent communication and interpersonal skills

Qualifications

• Master’s degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)
• Current CPR certification

Skills

• Strong clinical skills

• Proficient in using medical software and equipment

Pay

The pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation), with a tax-free stipend amount to be determined. The estimated weekly payment includes hourly wages, meal and incidental expenses, and housing expenses incurred on behalf of the company. Additional benefits such as medical, dental, and vision insurance are in addition to the weekly pay.

Schedule

Days schedule, 40 hours per week, 8 hours per day.
