Travel RN Case Management - $2,613 per week
HealthTrust Workforce Solutions is seeking a travel nurse RN Case Manager for an acute care case management assignment in San Leandro, California.
About the Role
Coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction, promotes continuity of care, and cost effectiveness through the integration of case management, utilization review, and discharge planning. Serves as a patient advocate through resource utilization, discharge planning, and addressing the holistic needs of the patient.
Responsibilities
- Comprehensive assessment, care coordination, and plan implementation for individual patient needs
- Ensure patient preferences are integrated into care plans
- Advocate for patients through resource utilization and discharge planning
- Collaborate with interdisciplinary teams (IDT)
- Utilize MCG guidelines for case management
- Manage care across acute care settings, including LTAC or STACH
Requirements
- Active California RN license (must be attached to resume)
- BLS certification through the American Heart Association (must be attached to submission packet)
- Minimum 2 years of recent RN Case Management experience
- Experience in LTAC or STACH settings
- Experience with MCG, discharge planning, and acute care case management
- Strong interdisciplinary team collaboration skills
Schedule
- 13-week assignment
- 40 hours per week
- 8-hour day shifts (Monday–Friday, 8:30 AM–5:00 PM)
