About the role
Deliver high-quality, compassionate nursing care for hospice patients.
Manage and implement individualized plans of care.
Serve as a floating clinician, temporarily assigned to different hospice branches based on census needs or staffing gaps.
Responsibilities
- Manage and implement individualized plans of care.
- Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
- Absolutely assess and address patients’ physical, emotional, spiritual, and environmental needs.
- Accurately document all care provided and ensure compliance with hospice protocols.
- Educate families and caregivers on care instructions, medications, and safety.
- Participate in the on-call rotation and provide after-hours care as needed.
- Collaborate with physicians, referral sources, and the care team.
- Aid with referrals, regulatory documentation, and payer compliance.
- Travel to various Gentiva hospice locations as assigned to fill urgent needs.
Qualifications
- Current, unencumbered RN license in the state of employment.
- Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting).
- Current CPR certification.
Preferred Experience
- Hospice and end-of-life nursing experience.
- Skilled in clinical assessments and compassionate communication.
- Strong critical thinking, flexibility, and problem-solving skills.
- Comfortable working in varied environments with shifting needs.
- Proficient in documentation and electronic health records.
- Calm and supportive in crisis situations; skilled at family engagement.
