Travel Registered Nurse – Hospice (Regional Support)

About the role

Deliver high-quality, compassionate nursing care for hospice patients.
Manage and implement individualized plans of care.
Serve as a floating clinician, temporarily assigned to different hospice branches based on census needs or staffing gaps.

Responsibilities

  • Manage and implement individualized plans of care.
  • Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
  • Absolutely assess and address patients’ physical, emotional, spiritual, and environmental needs.
  • Accurately document all care provided and ensure compliance with hospice protocols.
  • Educate families and caregivers on care instructions, medications, and safety.
  • Participate in the on-call rotation and provide after-hours care as needed.
  • Collaborate with physicians, referral sources, and the care team.
  • Aid with referrals, regulatory documentation, and payer compliance.
  • Travel to various Gentiva hospice locations as assigned to fill urgent needs.

Qualifications

  • Current, unencumbered RN license in the state of employment.
  • Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting).
  • Current CPR certification.

Preferred Experience

  • Hospice and end-of-life nursing experience.
  • Skilled in clinical assessments and compassionate communication.
  • Strong critical thinking, flexibility, and problem-solving skills.
  • Comfortable working in varied environments with shifting needs.
  • Proficient in documentation and electronic health records.
  • Calm and supportive in crisis situations; skilled at family engagement.
