Travel PT Home Health - Per Week

Role and Responsibilities

• Assess referred patients requiring rehabilitation services
• Develop individualized treatment plans
• Provide skilled physical therapy interventions according to physician orders
• Evaluate patient conditions
• Set realistic goals
• Document progress
• Educate patients and families on therapy methods
• Collaborate with multidisciplinary teams to plan and evaluate patient care

Qualifications and Skills

• Hold a Doctorate or Bachelor's degree in Physical Therapy with NPTE certification
• Graduated from an accredited program
• Possess at least one year of experience
• Current licensure or active application for licensure
• CPR certification
• Valid driver’s license
• Reliable transportation
