Requirements
- Active Oregon PT license (or Compact license in hand)
- Current BLS (AHA) certification
- Minimum 1 year skilled home health experience required; OASIS and Epic proficiency required
- Full COVID-19 vaccination (primary series plus 1 booster) required
- Personal vehicle required; IRS standard mileage reimbursement provided
