Details
Home health setting covering 13 counties in the St. Joseph, MO area, including Andrew, Atchison, Buchanan, Clay, Clinton, Daviess, DeKalb, Gentry, Harrison, Holt, Platte, and Worth counties
Rotating weekly coverage of farther counties, with adjusted patient loads on high-mileage days
Schedule is 5x8 days (8:00–16:30) or 4x10s depending on traveler preference
On-call required one Sunday every five weeks
Every fifth week, two weekday days off are exchanged for a Saturday and Sunday to complete admissions
EMR: Epic (required)
Requirements
- Minimum 1 year of PT experience required; home health experience is required
DPT degree required
Current MO state PT license required; pending licensure will be considered, but license must be active and in hand prior to start date — license required if applicable
BLS certification required
Previous travel experience preferred - Two professional references required for consideration
Copy of driver's license and Social Security card required for consideration
Traveler's permanent address must be at least 50 miles from the facility
Proficiency in OASIS documentation, plan of care, evaluations, reassessments, and goal-setting
Additional Information
- PTs perform their own Start of Care (SOC) assessments
Responsibilities include patient evaluation, treatment, home exercise program development, patient and family education, and safety and injury prevention
Skills utilized include gait training, assistive and adaptive device use, strength testing, vital sign monitoring, and skin integrity assessment
Scrubs required: solid black, grey, or khaki pants with a polo shirt or scrub top — no printed patterns
Previous facility employees must be separated for at least 1 year before returning as a traveler
Local candidates are accepted
RTO requests must be submitted at time of consideration
