Responsibilities

• Provide outpatient physical therapy services to adult patients in their homes
• Conduct start of care visits and follow-up assessments
• Document patient progress accurately and timely
• Travel to patient locations within a 75-mile radius, using reliable transportation
• Collaborate with the care team during periodic office meetings
• Maintain current state license, CPR, and BLS certification
• Adhere to agency policies and procedures, including liability insurance requirements
