The Good Life MedStaff is seeking a travel Home Health Physical Therapist for a position in Santa Rosa, California.

Responsibilities

• Provide physical therapy services to patients in their home environment.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and modify treatment plans as necessary.
• Document all treatments and patient progress in accordance with healthcare standards.

Requirements

• Current state licensure as a Physical Therapist.
• At least one year of relevant clinical experience.
• Passing score on the NCLEX-RN or equivalent.

Qualifications

• Experience working in a home health setting preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Skills

• Strong clinical judgment and problem-solving skills.
• Effective communication and documentation skills.
• Knowledge of home health care regulations and standards.

Pay

Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
