Description

Home health setting providing skilled visits under Medicare, Medicare Advantage, and Commercial insurance
Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
Home visit caseload of 6-8 skilled visits per day, minimum of 6 points daily
1-2 weekend shifts required per contract
Patient population includes med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADLs
Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems
Provided with laptop, MiFi for internet access, and portable printer

Requirements

• Current BLS certification required
• Active state PT license required if applicable
• Minimum 1 year of experience required, with home health experience required
• OASIS training preferred
• Valid driver's license and personal vehicle required for home visits
• Skilled in assessment, documentation, home exercise programs, patient/family education, and safety/injury prevention
• Knowledge of signs and symptoms of physical, sexual, and psychological abuse
• Cerner charting experience preferred
• Must be COVID vaccinated or have exemption on file
• Must have flu shot or declination, with mandatory mask wearing at all times if declined

Additional Information

Responsibilities include patient assessment, treatment planning, documentation, and billing across cardiopulmonary, orthopedic, and neurologic conditions
May float within the hospital system or to nearby affiliate facilities
Any solid color scrubs accepted
Smoke-free facility
Compliance documents due 10 days prior to start date
Pass/fail EKG exam allowed up to 3 attempts
Open to first-time travelers
Permanent employees must wait at least 1 year before returning as a travel employee
Return travelers limited to specific specialties including ICU, MICU, CVICU, ED, PCU, and MS/Tele
Local candidates accepted at the same pay rate
Must reside at least 100 miles from the facility to qualify for the position
