About the role
American Traveler is seeking a Physical Therapist for a 20-week travel Home Health position in Largo, Florida. The role requires 1+ year of home health experience and a valid Florida or compact PT license. Candidates must live at least 100 miles from the facility to qualify.
Responsibilities
- Provide skilled home visits for Medicare, Medicare Advantage, and Commercial insurance patients
- Manage a caseload of 6-8 home skilled visits per day (minimum 6 points daily, may increase by 1 at manager’s discretion)
- Assess and treat patients with med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL-focused conditions
- Develop home exercise programs and provide patient/family education
- Perform assessments, treatment planning, and documentation using BayCare Forms, BayCare Scheduling, Ace Timesheet, and Cerner (preferred)
- Self-schedule visits and hours based on HomeCare office needs
- Identify signs and symptoms of physical, sexual, and psychological abuse
- Ensure safety and injury prevention in home settings
- Use provided equipment: laptop, MiFi for internet access, portable printers, gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchairs
Requirements
- Minimum 1 year of experience, including home health
- OASIS training preferred
- BLS certification required
- Valid Florida PT license or compact license at time of submittal
- Valid driver’s license and car for home visits
- COVID vaccination or exemption form on file
- Flu shot or declination on file; mandatory mask-wearing if declined
- No requested time off (RTO) allowed during the contract
- First-time travelers will not be considered
