Travel Physical Therapist specializing in home health and hospice evaluations.
Responsibilities
- Provide therapy services to patients of all ages
- Conduct start of care and recertification visits
- Handle 5-6 visits daily
- Meet a productivity standard of 6.5 points per day
Schedule
Deliver therapy services Monday to Friday, 8am‑5pm, with guaranteed 40 hours weekly.
Qualifications & Skills
- Minimum 1 year of experience
- Home health experience preferred
- Knowledge of documentation and OASIS training preferred
- Capable of managing diverse patient needs
