About the role
Provide physical therapy services in patients' homes within Medford, Oregon.
Responsibilities
- Conduct PT evaluations, start of care, recertifications, resumption of care, and supervision visits
- Manage physical therapy visits in a home health setting
- Deliver quality physical therapy care in patients' homes
- Ensure compliance with regulations
- Coordinate with healthcare teams to support patient recovery and management of chronic conditions
Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution
- Active State License in Physical Therapy
- BLS certification (may be required)
- Prior home health experience

Schedule
40 hours per week, Monday-Friday, 8-hour shifts.
