Travel Skilled Nursing Facility (SNF) Physical Therapist providing rehabilitative services in a community healthcare setting.
Responsibilities
- Deliver direct patient care
- Develop treatment plans
- Document in compliance with facility standards
- Collaborate with interdisciplinary teams to promote patient recovery and functional mobility
Qualifications
- Valid state licensure as a Physical Therapist
- Graduation from an accredited program
- Minimum of 2 years experience in SNF or similar
- Proficiency with PointClickCare EMR
- Strong communication skills
- Experience with geriatric populations is preferred

Schedule
