Travel Physical Therapist - SNF / LTC - $2,328 per week

About the role

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Eugene, Oregon. The assignment is with a skilled nursing facility / long‑term care (SNF/LTC) setting, starting ASAP for a 13‑week contract, 40 hours per week on day shifts.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
• Incorporate the organization's vision, values, mission and critical goals into job performance.

Requirements

• Graduate of an accredited physical therapy program.
• Current state license as a Registered Physical Therapist (PT) or actively in the process of obtaining licensure.

Pay

Pay package is based on 8‑hour shifts and 40 hours per week (subject to confirmation) with a tax‑free stipend amount to be determined.
