Travel Physical Therapist - SNF / LTC - $2,135 per week

A great SNF / LTC in Covington, Louisiana is seeking an experienced or newly graduated Physical Therapist (PT) for a travel contract assignment. This is a contract position with a days schedule. The role assumes responsibility and accountability for incorporating the vision, values, mission, and critical goals of the organization into job performance. Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT, or be actively in the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
