Job Summary
The Physical Therapist will partner with residents and interdisciplinary care teams to deliver personalized physical therapy services aimed at optimizing residents' functional abilities and independence in activities of daily living.
Responsibilities
- Evaluate residents
- Set functional treatment goals
- Document care in the Electronic Medical Record
- Coordinate discharge planning
Requirements
- Bachelor's degree or higher
- Active license as a Physical Therapist in the applicable state
- Prior experience in a long-term care or skilled nursing facility preferred
About the Role
This role is based on a day shift schedule and requires 40 hours per week, starting on 08/03/2026. The Physical Therapist will perform evaluative and therapeutic procedures under the supervision of a physician or accepted healthcare provider, establishing and implementing a comprehensive treatment plan aligned with department protocols.
