Travel Physical Therapist Skilled Nursing Facility Job

About the Role

This is a temporary travel assignment in a skilled nursing care setting.

Responsibilities

• Evaluate residents to determine PT needs and develop individualized treatment plans
• Provide direct physical therapy interventions to improve mobility, strength, balance, gait, and functional independence
• Reassess progress, document findings, and adjust plans of care accordingly
• Educate residents and families on home exercise programs, safety, and fall prevention
• Collaborate with the interdisciplinary team to coordinate care and support discharge planning
• Maintain accurate patient records and ensure compliance with regulations and facility policies

Requirements

• Active Physical Therapist license in Maryland or eligibility for licensure by endorsement
• Doctor of Physical Therapy (DPT) degree from an accredited program preferred
• BLS/CPR certification preferred
• Minimum of 1 year of PT experience in skilled nursing or rehabilitation settings preferred
• Ability to work effectively in a SNF environment and comply with facility processes
• Strong communication, documentation, and teamwork skills

Pay

Weekly pay: $2,296–$2,436 per week. Guaranteed hours: 35 per week.

Schedule

Day shifts, five days per week (08:00–16:30).
