Travel Physical Therapist Skilled Nursing Facility Job

About the Role

Physical Therapist position in a long-term care / skilled nursing facility. This is a 13-week contract with a day shift schedule (8:00 a.m.–4:30 p.m.), 5 days per week (5x8).

Responsibilities

• Conduct PT evaluations and develop individualized plans of care for residents in a skilled nursing setting.
• Implement therapy interventions to improve mobility, strength, balance, gait, and pain management.
• Reassess residents, adjust treatment plans, and document progress and outcomes.
• Collaborate with an interdisciplinary team including nursing staff and rehab professionals.
• Document patient care in the electronic medical record and ensure compliance with privacy and facility policies.
• Educate residents and families on home exercise programs and activity modification.

Requirements

• Ability to work in a long-term care / skilled nursing facility setting.
• Availability to start on 08/31/2026.
• Able to commit to a 40-hour workweek.
• Strong communication and teamwork skills.
• Proficiency with standard electronic medical records and documentation.
• Willingness to follow facility policies and safety protocols.

Pay

Weekly pay: $2,419–$2,598 per week.

Schedule

• 40 guaranteed hours per week.
• Day shift: 8:00 a.m.–4:30 p.m., 5 days per week (5x8).
