Responsibilities

• Conduct initial and ongoing physical therapy evaluations for residents, establishing individualized treatment plans aligned with short- and long-term goals
• Administer therapeutic interventions, including gait training, therapeutic exercises, neuromuscular reeducation, balance training, and functional mobility activities
• Monitor progress, document outcomes, adjust treatment plans, and communicate changes to the interdisciplinary team
• Implement discharge planning and progress toward functional goals, collaborating with nursing, physicians, and therapy aides as needed
• Ensure compliance with regulatory standards, documentation requirements, and facility policies; participate in quality improvement and patient safety initiatives

Requirements

• Doctor of Physical Therapy (DPT) degree from an accredited program
• Active Physical Therapist license and eligibility to practice in Maine (or current licensure as applicable)
• Experience in or demonstrated comfort with a skilled nursing/geriatric rehabilitation setting
• Ability to participate in a travel assignment and meet the schedule requirements above
• Strong communication and collaboration skills; proficient in patient documentation and progress reporting
• Capacity to work the stated shift and maintain the guaranteed hours
