Travel Physical Therapist Skilled Nursing Facility Job

Responsibilities

• Assess residents’ physical abilities, mobility, gait, strength, and range of motion; develop individualized treatment plans
• Deliver evidence-based physical therapy interventions to improve functional independence and reduce risk of decline
• Reassess progress regularly; document outcomes and adjust plans to meet short- and long-term goals
• Collaborate with an interdisciplinary care team to coordinate care, communicate progress, and support discharge planning
• Provide patient and family education, including home exercise programs and safety strategies for daily activities
• Maintain accurate, timely documentation in charting systems and ensure compliance with regulatory standards

Requirements

• Doctor of Physical Therapy (DPT) degree
• Active Physical Therapist license in North Carolina or eligibility for licensure in North Carolina
• CPR/BLS certification (or willingness to obtain)
• At least 1 year of physical therapy experience in a skilled nursing facility or similar setting preferred
• Strong communication skills and ability to work within a multidisciplinary team
• Ability to work the 13-week assignment with guaranteed hours and a 5x8 schedule
