Travel Physical Therapist / PT - West Reading, PA
What You'll Do
- Provide skilled Physical Therapy services in accordance with clinical standards, physician orders, plan of care, and facility expectations.
- Evaluate, treat, document, and help patients improve mobility, function, independence, safety, and quality of life.
- Develop and update individualized treatment plans based on patient needs, goals, and progress.
- Collaborate with rehab leadership, nursing, facility teams, families, and care partners.
- Support discharge planning, patient education, and functional outcomes.
- Step into the assignment with professionalism, flexibility, and a patient-centered approach.
What We're Looking For
- Active or eligible Pennsylvania Physical Therapist / PT license.
- Prior skilled nursing, post-acute rehab, LTC, or geriatric experience preferred.
- Travel experience preferred, but not required for the right clinician.
- Net Health experience is a plus.
- Strong documentation, communication, and team collaboration skills.
- Ability to start quickly and complete required onboarding before assignment start.

Qualifications
