Travel Physical Therapist / PT - West Reading, PA

What You’ll Do

Provide skilled Physical Therapy services in accordance with clinical standards, physician orders, plan of care, and facility expectations.

Evaluate, treat, document, and help patients improve mobility, function, independence, safety, and quality of life.

Develop and update individualized treatment plans based on patient needs, goals, and progress.

Collaborate with rehab leadership, nursing, facility teams, families, and care partners.

Support discharge planning, patient education, and functional outcomes.

What We’re Looking For

Active or eligible Pennsylvania Physical Therapist / PT license.

Prior skilled nursing, post-acute rehab, LTC, or geriatric experience preferred.

Travel experience preferred, but not required for the right clinician.

Net Health experience is a plus.

Strong documentation, communication, and team collaboration skills.

Ability to start quickly and complete required onboarding before assignment start.

Guaranteed Hours: 35 hours per week

Estimated Weekly Gross Pay: $2,366
