Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized treatment plans
- Monitor patient progress
- Collaborate with care teams
- Educate patients and caregivers on exercises and home programs
Qualifications
- Active PT license or eligibility in Oregon
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience in SNF settings
- Preferred: Travel or contract therapy experience, Knowledge of SNF documentation and Medicare compliance
