Key Responsibilities

• Assess patients' conditions and create tailored treatment plans.
• Utilize Epic software for documentation and patient management.
• Perform therapy sessions during daytime shifts, typically 8.5 hours per day.

Qualifications & Skills

• Minimum 5 years of experience in home health care.
• Valid Physical Therapy License and current BLS certification.
• Proficiency with Epic software.

Must-Have Requirements

• BLS certification.
