Responsibilities
- Evaluate, develop, and implement personalized treatment plans to enhance patient mobility, strength, and overall quality of life.
- Congduct comprehensive assessments, provide therapeutic interventions, monitor progress, and adjust treatments as needed.
- Educate patients and caregivers on rehabilitation goals and home programs, collaborate with healthcare teams, ensure compliance with standards, and maintain accurate documentation.
Qualifications & Skills
- Active state license
- Graduate of an accredited PT program
- Strong assessment and treatment planning skills
- Excellent communication and documentation abilities
- CPR/BLS certification preferred
- Experience in rehab, SNF, outpatient, or acute care settings is advantageous
