Role & Responsibilities
- Provide skilled home health physical therapy services to adult and geriatric patients in Owensboro, KY.
- Conduct assessments, develop and implement individualized care plans, perform home safety evaluations, and educate patients and caregivers on mobility, safety, and exercise programs.
- Collaborate with an interdisciplinary team to optimize patient outcomes and complete accurate documentation via EMR.
- Manage a caseload with diverse diagnoses, focusing on gait, balance, strength, fall prevention, and functional mobility.
- Most visits are local in a supportive, team-oriented environment that encourages professional growth and community engagement.
Qualifications & Skills
- 1 year of home health physical therapy experience
- Valid PT license (compact accepted)
- SSN and DOB required
- A reference
- Driver’s license and auto insurance
- PREFERRED: Experience with Homecare Homebase documentation
Key Duties
