• Collaborate with healthcare team members to ensure optimal patient care and document progress accurately.

Qualifications and Skills

• Valid Physical Therapist license, experience in SNF settings preferred.
• Strong clinical assessment skills, excellent communication, and ability to work independently.
• Knowledge of rehabilitation techniques and documentation requirements.
